In keeping with the policies set forth in the Handbook for Athletics 2011, student athletes and their parent(s) are required to complete the sports participation agreements in the link posted below. These include a Child/Minor Acknowledgement Form, Student Athlete Code of Conduct and Parent/Guardian Code of Conduct.

These documents replace the medical waiver and hold harmless agreement used previously.
